How CBD Creams and Balms Work: A Guide to Topical CBD

How CBD Creams and Balms Work: A Guide to Topical CBD - CBD education article by CJ's Medicine Cabinet

Not all CBD products are designed to be swallowed. Topical CBD products like creams, balms, salves, and lotions are applied directly to the skin, where they interact with cannabinoid receptors in the local area without entering the bloodstream. This makes them ideal for targeted relief.

How Topical CBD Works

Your Skin Has an Endocannabinoid System

Your skin is the largest organ in your body, and it has its own endocannabinoid system (ECS). CB1 and CB2 receptors are found in skin cells, hair follicles, sebaceous glands, and sensory nerve fibers. When you apply CBD topically, it binds to these local receptors.

It Stays Local

Unlike CBD oil taken under the tongue or in a capsule, topical CBD does not enter your bloodstream in significant amounts. It penetrates the outer layers of skin and interacts with receptors in the immediate area. This means the effects are concentrated where you apply it.

Absorption Factors

Several factors affect how well topical CBD absorbs:

  • Skin thickness: Thinner skin (wrists, temples, neck) absorbs more readily than thicker areas (palms, soles)
  • Carrier ingredients: Products with penetration enhancers like menthol, camphor, or emu oil help CBD reach deeper layers
  • Application method: Massaging the product in improves absorption compared to light application
  • Skin condition: Clean, exfoliated skin absorbs better than dry or calloused skin

What Topical CBD Helps With

Joint and Muscle Pain

This is the most common use for topical CBD. A 2016 study in the European Journal of Pain found that topical CBD reduced joint swelling and pain behaviors in arthritic rats without side effects. Human studies are ongoing, but anecdotal evidence is strong.

Inflammation

CBD's anti-inflammatory properties are well documented. Applying CBD directly to inflamed areas (sore muscles, swollen joints, minor injuries) delivers the anti-inflammatory compound right where it is needed.

Skin Conditions

Research suggests CBD may help with several skin conditions:

  • Eczema and dermatitis: CBD's anti-inflammatory properties may reduce redness, itching, and irritation
  • Acne: CBD has been shown to reduce sebum production and has antibacterial properties
  • Psoriasis: CBD may slow the overgrowth of skin cells that causes psoriatic plaques
  • Dry skin: Many CBD topicals include moisturizing ingredients alongside CBD

Localized Nerve Pain

For conditions like neuropathy, carpal tunnel, or sciatica, applying CBD to the affected area may provide localized relief by interacting with pain-sensing receptors in the skin.

Types of Topical CBD Products

Creams and Lotions

Lighter consistency, absorb quickly, good for everyday use and larger areas. Best for skin conditions and mild discomfort.

Balms and Salves

Thicker consistency with waxes or butters. They create a protective barrier and release CBD over a longer period. Best for joints and targeted pain relief.

Roll-Ons and Sticks

Convenient, mess-free application. Good for on-the-go use. Many include cooling menthol or warming capsaicin.

Transdermal Patches

These are designed to push CBD through the skin and into the bloodstream. They provide slow, steady delivery over 8 to 12 hours. These are technically different from standard topicals because they do have systemic effects.

How to Use Topical CBD

1. Clean the area where you plan to apply

2. Apply a generous amount and massage it in thoroughly

3. Wait 15 to 30 minutes before expecting to feel effects

4. Reapply as needed, typically every 4 to 6 hours

5. Be consistent for chronic conditions. Daily application builds cumulative benefits

What to Look For

  • CBD concentration: Look for products that list the total mg of CBD, not just "hemp extract"
  • Full spectrum vs. isolate: Full spectrum topicals include additional cannabinoids and terpenes that may enhance the effect
  • Additional ingredients: Menthol, arnica, turmeric, and capsaicin can complement CBD's effects
  • Third-party testing: Certificates of Analysis should be available
